Our dedicated team provides adult day health services across three counties: Jackson, Lenawee and Hillsdale

Our interdisciplinary team of healthcare professionals will work together with you and your family to develop the most effective plan of care for you.

Our Interdisciplinary Team

The Thome PACE Care team includes health care professionals who know how to manage the care of the elderly. The team creates a personal health care plan for every participant. This plan will help monitor changes in health care needs. It also helps manage all health care services and provide support for participants in the home and our Center. The team includes:

  • Doctors

  • Nurses

  • Personal care providers

  • Home health providers

  • PACE Center Manager

  • Occupational and physical therapists

  • Social workers

  • Activity staff

  • Dietitians

  • Transportation Coordinator 

  • Chaplain Services
