PACE provides the nursing home alternatives Michigan needs
Our mission is to promote the wellness, dignity, and independence of older adults by providing high quality, innovative residential & supportive services.
The primary goal of Thome PACE is to provide members of the community the care and medical treatment they need in order to help them live in their homes for as long as possible.
We achieve this by offering a comprehensive package of services delivered by a team of specialized and caring healthcare professionals.
History
Thome PACE was established in 2016 by Brio Living Services and WellWise Area Agency on Aging, nonprofit organizations dedicated to the health and wellbeing of older adults in the communities they serve.
Thome PACE is located in Jackson, MI and serves older adults age 55 or older who have ongoing healthcare needs and live within our service area. (See the Thome PACE Service Area Map)
