ELIGIBILITY

Am I Eligible for PACE?

You can join Thome PACE if you meet the following conditions:

​

✔ You are 55 years old or older.

​

✔ You live in Jackson, Hillsdale or Lenawee County.

 

✔ You meet level of care requirements as determined by the State of Michigan

 

✔ You are able to live safely in the community with the support of PACE® services when you join.

​

​

A potential PACE enrollee may be any or all of the following:

​

  • Entitled to Medicare Part A

​

  • Enrolled under Medicare Part B

​

  • Eligible for or enrolled in Medicaid
